In order to receive your extended 5yr labor warranty for newly installed HVAC equipment, please fill out and submit the form below. You will have 90 days from the date of install to submit this form. Once submitted, the form will be received and review by our office. If the information provided below matches our records, we will update the information in our system. Once updated, our office will contact you via email and/or phone to inform you of the approval and updated information. Please be advised labor warranties are voided if failure of a component is a result of neglected annual maintenance, or refusal to replace failing components that are recommended by a certified service technician during an annual maintenance visit. Labor warranties are not valid for equipment installed and/or serviced by another company.

I agree this form is being submitted within 90 days of an install of my HVAC system performed by Carr Electric Heating & Cooling. I understand once I submit this form, the above information will be received & reviewed by Carr Electric Heating & Cooling and cross-referenced by their records. Once approved, I will be notified via email and/or phone call from Carr Electric Heating & Cooling of the approval and update to my file. I also understand my labor warranty is voided if failure of any of the components on my furnace/air handler, air conditioner/heat pump or evaporator coil are a result of neglected annual maintenance, or refusal to replace failing components recommended by a certified service technician during an annual maintenance visit.
